What’s involved?
Food and Health Record
(Submit at least 48 hours prior to visit)
You will be asked to provide details about the foods you eat, your medical history, your activity level and any medication or supplements being used. This allows us to get to know you and identify any obvious areas for improvement or change.
Nutritional Analysis of Food Record Using Nutrition Software
A comprehensive computerized nutritional analysis of your current food and nutrient intake using the food records completed in your food and health record. We will provide you with a detailed computer report of your usual intake including calories, protein, fat, carbohydrate and vitamins/minerals.
Client Consultation (Approximately 60 minutes)
During this visit, we will review your food and health record and obtain any additional information needed to complete the consultation. Recommendations for change will be made and goals will be set to achieve these recommendations. As a team, we will determine the best strategies to meet the established goals.
Follow Up Package (Mail out)
The follow up package will include a summary of your consultation and review of the recommendations made during the visit. We will also provide any pertinent educational handouts to help achieve your goals.
